Today, we're joined by Dr. RS Madhavi, a senior consultant pediatrician here in Hyderabad with over two decades of experience. Dr. Madhavi is passionate about providing the best care for children, from newborns to older kids. Her expertise in newborn care, allergies, asthma, and nutrition is invaluable to families in Hyderabad. She's currently practicing at the Birthplace Clinic by Cloud9 in Gachibowli. 1. A Day in the Life of a Hyderabad Pediatrician
Madhavi Adimulam: Dr. Madhavi, what does a typical day look like for you as a pediatrician in Hyderabad? What are some of the joys and challenges you encounter in your work?
Dr. RS Madhavi: There's no typical day, that's the beauty of it! Some days, I'm rushing to the hospital for deliveries, then it's off to my clinic in Gachibowli from 10 am to 2 pm. After that, I’m at the Banjara Hills Unit Hospital until the evening. Every other week, I'm on call, attending to deliveries and caring for sick children throughout the night. My days are truly defined by my patients and, of course, my children at home.
2. The Rewards of Pediatrics
Madhavi Adimulam: What are some of the most rewarding or inspiring moments you've experienced in your career as a pediatrician?
Dr. RS Madhavi: There are so many rewarding moments. Witnessing the resilience of children is truly inspiring. They bounce back from illness so quickly, bringing immense joy. Making an early diagnosis, guiding a patient away from a life-threatening illness, or seeing parents achieve positive results by following my advice – these are all heartwarming experiences.
3. Advice for Aspiring Pediatricians in Hyderabad
Madhavi Adimulam: What advice would you give to someone in Hyderabad who's considering a career in pediatrics or rehabilitation in pediatrics?
Dr. RS Madhavi: Pediatrics, neonatology, and other pediatric subspecialties require immense commitment. Here are a few key things to consider:
- Strong Knowledge and Continuous Learning: Stay updated on the latest advancements in child healthcare.
- Exceptional Communication Skills: Build trust with families and convey information clearly and empathetically.
- Embrace Challenges: Be adaptable and resourceful in finding solutions to unexpected situations.
- Work-Life Balance: Prioritize self-care and find ways to maintain a healthy balance between your professional and personal life.
Madhavi Adimulam: You put it beautifully. It sounds like aspiring pediatricians need to build confidence in families through communication, embrace challenges with creativity, and work hard while maintaining a balanced life. Now, let's shift our focus to a topic that's close to both of us: child development.
4. Early Identification of Autism and Developmental Delays in Hyderabad
Madhavi Adimulam: As a pediatrician in Hyderabad, you play a crucial role in the early identification of developmental delays, including autism. What are some of the subtle or not-so-subtle signs that trigger your concern? And once you have flagged these concerns, how do you typically guide parents towards the right specialists for autism diagnosis and treatment in Hyderabad?
Dr. RS Madhavi: Early diagnosis is crucial for supporting children with autism. Here are some early signs parents should be aware of:
- Not responding to name-calling by one year of age.
- Lack of eye contact.
- No stranger fear.
- Limited social interaction.
- Repetitive movements like flapping hands, circling, or rocking.
- Delayed speech.
- Hyperactivity.
I closely observe children during their visits. If I suspect autism, I ask parents specific questions about the child’s behavior and development during every vaccination visit. By 18 months, if I see a lack of improvement, I advise parents to consult a specialist like a psychiatrist, behavioral pediatrician, or psychologist. After their evaluation, I encourage parents to follow the recommended therapies.
5. Addressing Parental Resistance
Madhavi Adimulam: You mentioned that many parents resist seeking help for developmental concerns, especially when it comes to mental health. How do you encourage them to take that crucial step?
Dr. RS Madhavi: Over 90% of parents show resistance, it’s a significant challenge. Seeking help from a mental health professional is still considered taboo in India. In my early years, I would gently suggest it a few times. Now, I'm more assertive. I’ve seen parents regret not taking action sooner. I remind them that:
- Early intervention is more effective and often requires fewer therapy sessions.
- Early support can help children keep pace with their peers when they start school.
- Addressing challenges early can prevent feelings of inferiority and other difficulties later on.
I emphasize that autistic children are often intelligent and have immense potential for a good life, career, and relationships.
Madhavi Adimulam: That's a powerful message, Dr. Madhavi. Starting therapies early can make a world of difference. Have you noticed a shift in parental attitudes over the years?
Dr. RS Madhavi: Definitely. Now, I plant the seed early on, even if parents are resistant. They often go home and research, becoming more aware of the potential challenges and the benefits of intervention. Awareness is the first step toward seeking the right help.

6. The Pediatrician's Continued Role
Madhavi Adimulam: Beyond the initial referral, what is the continued role of a pediatrician in supporting families of children with autism?
Dr. RS Madhavi: A pediatrician’s role extends far beyond the referral. We provide ongoing support to parents navigating the complexities of therapy, finances, and the emotional rollercoaster of raising a child with autism. We address their concerns about:
- The child’s progress.
- Potential infections picked up at therapy centers.
- Difficulties adjusting to therapy.
- Ongoing vaccinations.
- Viral and other infections.
- Sleep issues (for which we refer them to sleep therapists).
- Constipation or food allergies.
We are their constant source of support and reassurance.
7. Parental Reactions and Support Strategies
Madhavi Adimulam: What are the common reactions you see from parents in Hyderabad when their child is first diagnosed with autism or any developmental disorder? How do you offer support and guidance during this difficult period for them?
Dr. RS Madhavi: Guilt and blame are common reactions. Parents often blame themselves or each other. They may be unsure about the effectiveness of therapies. I offer reassurance and hope. I let them know that a diagnosis is not a dead end. Early intervention and consistent therapy can make a significant difference. I share stories of other families who have successfully navigated this journey. I also guide them to support groups and resources, connecting them with others who understand their experiences. It's crucial to provide a listening ear, validate their feelings, and empower them to take positive action.
